To receive a No-Cost CaptionCall phone, contact our office to obtain the necessary certification verifying that you have hearing loss and face challenges hearing on the phone.

To qualify, you must meet the following three CaptionCall requirements:
a. Have a hearing loss that requires the use of captioned telephone service.
b. Have access to a cell phone or a standard home phone connection.
c. Provide a Professional Certification Form verifying your hearing loss.
